Back pain is common. Chronic low back pain and sciatica are even more common. MRIs often look dramatic. Symptoms can feel dramatic. But in most cases, the spine is not broken, unstable, or doomed.

We take a practical, evidence-informed approach to back pain that sits firmly within Personal Training and Lifestyle Medicine. We are not physiotherapists and we do not replace medical care. What we do is help people rebuild strength, fitness, and confidence in a way that supports long-term spinal health.

Chronic Low Back Pain Is Usually a Capacity Problem, Not a Structural Failure

Modern research shows that most persistent low back pain is not caused by catastrophic instability or fragile discs. Many people with “abnormal” MRIs have no pain at all. Others with severe pain have minimal structural findings.

Sedentary lifestyles, stress, disrupted sleep, reduced conditioning and fear of movement all reduce the body’s load tolerance. When that tolerance drops, normal life can begin to feel threatening to the back.

Not “protect” a fragile spine.

Strength Training for Back Pain: Whole Body First

We do not obsess over tiny spinal stabilisers. We do not chase perfect posture. We do not avoid bending forever.

Instead, we focus on:

  • Strengthening hips, legs, trunk and upper body
  • Building aerobic capacity
  • Gradually reintroducing real-life movement
  • Improving body composition where appropriate

The spine works as part of a system. When the system becomes stronger and more conditioned, low back pain and sciatica symptoms often reduce.

This is not about a magic core exercise. It is about intelligent progression.

Graded Exposure, Not Bracing for Life

Many people with chronic back pain have been told they are “unstable” or “weak in the core.” That narrative can create more fear than benefit.

At DrTLifestyle we use a graded approach:

Longstanding, stable sciatica is common. Old disc bulges are common. Many remain unchanged for years.

Provided there are no red flags or progressive neurological symptoms, strength training and conditioning are often not only safe but beneficial.

We progress from supported movements to free weights when appropriate. Confidence increases alongside strength. Flare frequency often reduces.
